How do I submit an amendment or continuing review (renewal) to an approved project?

1. Complete the IRB Continuing Review/Amendment Form (Word)
Submit the request
Submit the application via e-mail to IRB@uga.edu or
by campus or postal mail to*:

What materials should I submit to the Human Subjects Office?

• A fully completed IRB Continuing Review/Amendment Form
• Any new or revised instruments or materials (e.g., surveys, interview guides, or recruitment materials or script)
• Any new or revised consent documents
• Proof of completion of IRB (CITI) training for a new researcher; if new researcher will assume the role of PI, the first two pages of the application (with the signature of the new PI on the second page)
